Immunization Compliance Policy
Attention all first time Pittsburg State University students:
- Complete the Proof of Immunization Compliance Form. This form must be completed by your health care provider OR an official certified copy of your immunization records must be attached to this completed form. For more information, please contact the Student Health Center: healthctr@pittstate.edu.
- IMPORTANT: The completed Immunization Compliance form MUST be submitted before you will be permitted to enroll in classes.
