Health Savings Account (HSA) & Health Reimbursement Account (HRA) Members receive new Optum payment cards

Optum Credit Card

All Health Savings Account and Health Reimbursement Account members will receive their new Optum payment cards soon!

The new Optum payment cards will arrive throughout February and members will have them in hand by March 2, 2016, the date they will become active.

Here is what you need to know about the new cards:

  • All cardholders will receive a new card replacing their current US Bank card.
  • The new Optum payment card will function the same as the US Bank card.
  • There is a 1-day period on March 1 when neither payment card will work. This is necessary to ensure that the new cards are functioning properly. If this creates an inconvenience for any member, they can call the customer service number on the back of their new Optum payment card to discuss alternative options.

Key Dates

  • February 29, 2016: Member's current cards will work through the end of this day. After this date, current US Bank cards will no longer work.
  • March 1, 2016: On this day, neither the US Bank card or the new Optum payment card will work to ensure the new Optum payment cards are functioning properly
    NOTE: If this creates an inconvenience for any member, they can contact the customer service number on the back of the Optum card.
  •  March 2, 2016: Employees will be able to use their new Optum cards beginning on this date.

2015 W-2 Forms

W-2 forms for calendar year 2015 are available electronically through the State of Kansas Employee Self Service Portal.  This option allows you to securely receive your W-2 electronically.  

If you consented to receive your W-2 electronically last year, that consent is still in effect.  If you would like to withdraw that consent and have a W-2 mailed to you, you must log onto the Employee Self Service Portal to withdraw consent.   Employees that would like to add electronic consent must log onto the Employee Self Service Portal and consent to receive this information electronically.

Please note, printed W-2 forms, for employees who do not consent to receive their W-2 information electronically in ESS, will be mailed the last week of January in accordance with IRS regulations.

2015 IRS 1095-C Forms

The State Employee Health Plan (SEHP) requested that we provide information about the 1095-C Form.

The State of Kansas is required to provide the 1095-C Form to all employees who were eligible for coverage in the State Employee Health Plan (SEHP) for one or more months of the calendar year.  This Form is provided to all eligible employees, even those who declined or waived SEHP coverage in 2015.  The 1095-C Form is similar to W-2 or 1099 Forms that you will need to retain for your tax records and the preparation of your tax return.  More information on the 1095-C Form may be found on IRS websites here and here.

Your 1095-C Form is available in the state of Kansas Membership Administration Portal (MAP).  This portal is used by employees to make SEHP open enrollment elections and to request mid-year changes.  You will use your PSU active directory ID and password to sign in to MAP.  Once logged in, the 1095-C Form is located under the “Forms” tab, under “Tax Forms.”  The SEHP stated they also will mail this Form to the primary member covered under the SEHP at that member’s preferred mailing address which is located in the “Member & Family” tab in MAP.

Employees only need to sign up for 1 screening during the period – November 16, 2015 through November 15, 2016.  Log in to your account on the wellness portal at www.KansasHealthQuest.com and click on "Biometric Screenings" from the left menu and then select "Onsite Screenings." If you have problems with their online scheduling system, please call their Help Desk to schedule an appointment at 1-888-275-1205, Option 5.

Here are the dates for the Biometric Screenings on the PSU Campus:

  • February 16, 2016 - 7am-12noon - PSU, Overman Student Center (302 East Cleveland, Pittsburg, KS 66762) in the Governors Room

  • May 3, 2016 - 7am-12noon - PSU, Overman Student Center (302 East Cleveland, Pittsburg, KS 66762) in the Governors Room

  • May 4, 2016 - 7am-12noon - PSU, Overman Student Center (302 East Cleveland, Pittsburg, KS 66762) in the Governors Room

  • August 25, 2016 - 7am-12noon - PSU, Overman Student Center (302 East Cleveland, Pittsburg, KS 66762) in the Governors Room

  • August 26, 2016 - 7am-12noon - PSU, Overman Student Center (302 East Cleveland, Pittsburg, KS 66762) in the Governors Room
