The shingles vaccine would be covered under the medical plan (NOT the Caremark Prescription Drug Plan).
The shingles vaccine would be covered at 100% as an immunization. The shingles vaccine has a very low shelf life, and the member would have to pick it up at the pharmacy, as the doctors usually do not have this in their office.
The member would need to pay for the vaccine and then file a claim to Medical plan provider (Blue Cross/Blue Shield, Coventry, Preferred Health or UMR United Health) for the cost of the vaccine. The provider would bill the plan for the cost of the injection.
It is important that members understand that they need to file this under the Medical plan because Caremark does not cover drugs that require deep muscle injection such as this one.
